Criteria for RoH community members:

1. Community members who have regularly attended RoH’s community events
2. Individuals / families with limited community / family support
3. Has a senior in each of these households

Brief background: Client is single, full-time caregiver to her elderly parents. Both are 74 years old and suffer from multiple chronic conditions. Dad has HBP, diabetes and goes for dialysis thrice a week (end stage renal failure). He has angioplasty on his foot and travels on a motorised wheelchair. Mum was a stroke patient and has HBP, cholesterol and functional dyspepsia.
